When Seconds Count: Pole-Top and Manhole Rescue

A lineman aloft brushes an energized conductor and goes limp in the bucket. The two-person rule did its job: there is a qualified worker on the ground watching it happen. Before you read on, decide two things. What is the ground worker’s very first move, and how fast does that victim have to be on the ground? Picture the clock running while you answer.

Here is the hard number. The brain starts taking irreversible damage from oxygen loss somewhere around 4 to 6 minutes. That is why pole-top rescue is not a thing you figure out in the moment, it is a drilled skill practiced until the whole sequence runs in under about 4 minutes. The order is fixed: confirm the contact is clear so you are not the next one hit, lower the bucket or the victim to the ground, then begin CPR. Compressions in a bucket do not work, so the victim has to come down.

Now change the scene. A coworker collapses at the bottom of a manhole or vault and is not moving. Every instinct says climb down and drag them out. That instinct is what kills rescuers. The atmosphere or hazard that put them down is still there, and it does not care that you have good intentions. Most confined-space rescue deaths are the would-be rescuers who rushed in.

So the manhole answer is the opposite of the pole-top answer in one way: you do not go in. You call for help, you ventilate the space, and you pull the victim out with a retrieval line from above, after the atmosphere is tested. The rescue is just as urgent, but it runs from outside the hole.

Both scenes come down to one rule that sits above the rest. Do not become the second victim. A rescuer who rushes in unprotected does not save anyone, they just hand the next crew two bodies to recover instead of one. Train the rescue, keep your head, and bring everyone home.
